Appointments
Telephones are primarily attended by an experienced answering service
Monday through Friday from 7 am to 4 pm. If you have a medical emergency,
please go to the nearest emergency room for medical attention.
We use a scheduling strategy that leaves appointment times open for
patients who are sick and need to be seen either the same day or the next
business day. A well person physical, for example, will not normally be
scheduled in these urgent windows. There will clearly be some scaled
decisions regarding urgency and we may ask for your flexibility.

If you are a new patient, please print and complete both the Patient
Registration Form and the Medical History Form. Links to these
